Too much anesthetic is not a good idea: with complete numbness, drilling may be deep to injure the nerve underneath (Fig.5 N). To compromise, a small and short implant is placed at the site of #21, as compared to that at #20 (Fig.6).
At #21, the extraction socket (black area) is larger than the implant (white outline). If the implant is short, it should not be stable.
In contrast, at #20, the hole formed by drilling (black) is smaller than its implant. The latter, once getting in, must be stable.
