Open bite is aggravated probably due to bracket design/placement and wire manipulation.  While the first step is to re-bracket UL 3, no rebracketing is done for UR3 (Fig.1, as evidenced by the upper .016 ss wire being straight).  The discrepancy of bracket placement is not too obvious between the canine and the incisors on the right.
