Osteotomy is being enlarged with a 3 mm reamer (Fig.5).  Although there is bone height deficiency, the bone density is quite high.  Autogenous bone is being harvested while the reamers (2.5 and 3 mm) are being used.  Osteotomy depth is 9.06 mm.

A screw-type tapered implant is placed with insertion torque >60 Ncm (Fig.6: 4x14 mm).   The implant is not placed as high as the last reamer (3 mm in diameter, Fig.5).  There is no gag apical to the implant.  The possible explanation is that the bone is being pushed to the most apical region created the last reamer while the implant is being inserted.  The socket is partially obliterated by the implant (Fig.6).  The patient returns for final restoration 10 months postop.  There is bone regeneration and re-mineralization (Fig.10, compare to Fig.6).  A: abutment.
