Modified Screw-Retained Crown

A modified screw -retained crown is a cement-retained one with an occlusal access hole.  After cementation with removal of cement from the access (Fig.1), the crown and abutment is removed from the underlying implant (Fig.2) to remove residual cement from the crown/abutment junction.  Then the crown/abutment is fixed in place with the help of abutment screw (Fig.3).  The access is sealed with composite.  If the patient notes the short crown after crown delivery, the crown/abutment complex can be returned to lab for addition of the buccal extension (Fig.2' yellow line) to improve cosmetics (Fig.3').  If the cosmetic issue is noticed around impression, the lab will be reminded to make the buccal extension.

If the cosmetic problem is noted after cementation of a cement-retained crown, the crown may have to be redone.
