A 61-year-old man cracks the tooth #4 while chewing on a piece of chicken bone (Fig.1,2). The neighboring teeth have long roots. He must be a bruxer. An implant could be 16 or 18 mm long. After use of 3 mm reamer for 19 mm, a 3.8x15 mm dummy implant has difficulty in entering the osteotomy. But after use of 3.5 mm reamer for 15 mm, the dummy has low stability. So a 4x16 mm implant is anticipated.
