The 3rd Immediate Implant in a Row

A 62-year-old man will return for #3 immediate implant, 2.5 years after #2 one and .5 year after #1 one (Fig.1).  It appears that the available bone for the implant is the septum, which is not large.   The implant is not expected to be large. Sinus lift is anticipated to obtain additional stability.  Prepare osteotomes and bone expander kit.  Since the bony defect is extensive (*), prepare more larger size of allograft than the small one and Osteogen.  The amount of graft should be large, looking at the socket after extraction.  The antibiotic will be Clindamycin.  Use amalgam carrier for sinus lift and larger one for the rest of grafting.  To keep the graft in place, try a large abutment, e.g., 7.5 mm.  If there is severe gingival recession, draw blood for PRF.  Take preop photos.
