The tooth #3 has severe periodontitis with probably endodontic lesion (Fig.2). The tip of Gutta percha point from buccal apical fistula points to the buccal apices.
A 7x17 mm immediate implant is planned (Fig.3). The large implant will basically obliterate the enlarged socket to reduce the amount of bone graft and facilitate wound healing. The apical end of the implant will be engaged to the sinus floor to increase primary stability.
