Implant to be Placed at the Level of Crest and Septum

The tooth #15 of a 56-year-old man appears to be non-salvageable due to the large distal cervical caries (Fig.1 (CBCT) C).  Immediately after extraction and socket debridement and antibiotic soaking, a 2 mm pilot drill is used for 4 mm, followed by 2.8 and 3.6 mm round burs for 4-6 mm (Fig.1,2 (coronal section)).  At #14, the initial depth for 2 mm and 2.8 mm drills is 7 mm.  The depth may increase until 9 mm (Fig.1,3).  Insert parallel pins after initial osteotomies; check with surgical stent and/or take PA.  After placement of allograft/Osteogen, place 4x10 and 6x8 or 10 mm implants at #14 and 15, respectively.
