Bruxer and Infra-occlusion

A 73-year-old man is a bruxer with partial edentulism.  Before the last implant osteointegrates, a central incisor fractures (Fig.1(axial section) ^, 2 (sagittal) *, d (coronal) (CT taken 6 months earlier)).  In fact the last implant needs redo probably due to micromovement during healing period.  Therefore an immediate provisional at the lower incisor should be infra-occlusal.  The socket will be treated with Metronidazole.  Take preop photos.  Start osteotomy lingually so that the implant will engage to the mesiolinguodistal walls for primary stability (Fig.1',2').
