After repeated trial, a 6.5x12 mm implant is placed with insertion torque between 50 and 60 Ncm (Fig.10).  It is a little deeper than planned (Fig.5). A long cuff cemented abutment (6.8x4(4) mm, A in Fig.11) is placed for an immediate provisional.

Fig.11: 1.5 months postop; *: bone graft around the implant plateau.  Bone density increases significantly around the coronal portion of the implant 2 years 9 months post cementation (Fig.14 arrowhead).  This is partially due to unilateral mastication, since the tooth #31 is missing.  An implant has been recommended since the 1st appointment.
