After insertion of a 5.7x5.5(3) mm abutment, Vanilla Graft is placed (Fig.5 * with 2 mm buccal gap).  The abutment with a provisional is loose 4 months postop (Fig.6).  The abutment screw is loose and fractured 8 and 11 months post cementation, respectively (Fig.7).  The fractured screw seems to be loose within the implant well and is easily re-winded out.
