When the tooth #18 is extracted, the buccal and lingual plate are absent.  Initial osteotomy is established in the middle of the socket (Fig.1, including buccolingually).  A 1.6x7 mm guide pin is placed with approximately 3 mm new bone.  The apical tip of the guide pin is ~ 3 mm from the superior border of the Inferior Alveolar Canal (red dashed line).
