Place 2 or 3 Immediate Implants Involving Two Arches at the Same Time?

A 63-year-old man (CZ) is concerned about the missing tooth at #13 and requests an implant at the site only (Fig.1 (pan taken while implant is being placed at #19)).  There are two issues.  The first is associated with severe supraeruption of the opposing tooth #20.  The second issue is that there is infection involving #14. 

A reasonable approach is implants for #13 and 14 (Fig.2,  Clindamycin) and occlusal reduction of #20.  Start osteotomy with 2 mm pilot drill at #13 before extracting #14 so that it is easy to control osteotomy position and trajectory.  UF long implants will be chosen because of height.
