A 2 mm round tapered (RT) osteotome (Fig.3) was used to initiate osteotomy at the depth of 11 mm from the gingival level (7 yr 9 m post ext). 

The trajectory was later corrected by using a Lindermann bur. A 2 mm pilot drill extended osteotomy 14 mm deep.  Then a 3 mm RT was used to create osteotomy 14 mm deep.

Last a 4 mm RT osteotome enlarged the osteotomy 11 mm deep (Fig.4).
