Overdue Guides Still Work M

Incision is made at #2 and 3 because of bone graft palatal to #2 after implantation and 6-month-old guides (coronavirus pandemic).  The implant at #3 is first placed with precision; there is no hesitation with that at #2.  The palatal bone defect is confirmed.  With autogenous bone placed around the implant/abutments covered by pieces of collagen membrane, the wound is closed with 4/0 Chromic gut suture.  Since the keratinized tissue is wide at #31, osteotomy starts with a tissue punch.  The implant placement level is noted accurate clinically and radiographically (Fig.1,2).  The 3 implants have the same size (5x10 mm).  All of the sites use 6.0 mm bone profile drill before healing abutment insertion.  The surgery turns out to be smooth and streamlined.  Abutments are placed 5 months postop (Fig.3,4).  The upper crowns are redo twice because of light occlusion and proximal contact and finally are cemented 9 months postop with torque at 20 Ncm.
