After the last drill, use 4.5x17 mm Tatum tapered tap to 14 mm free hand.  If excessive torque is encountered, place the guide and use 4.5x7.3 mm drill.  If there is no bone loss 4 months postop, no pressure in the crest will prove to be a critical factor.  Take BW to confirm that threads are subcrestal.
