GEM Cap

A 55-year-old man presents to clinic with nocturnal pain associated with the tooth #18 (Fig.1).  After extraction and removal of granulation tissue, mineralized cortical allograft (particle size: 125μm – 850μm)) hydrated with .5 ml of .3 mg/ml of rhPDGF-BB (one component of GEM21S) is placed in the sockets with a thin septum (Fig.2 *).    Periodontal glue is applied to the top layer of the bone graft (Fig.3 blue curved line).  It appears that the bone graft becomes less mobile.  GEM Cap RCM (regenerative collagen membrane, 9-12 month resorption time) is placed with the glue applied to the periphery of the cap, which seems to be quite stable (Fig.4).  No suture is needed.
