New Beginning

A 74-year-old woman is nervous with possible extraction and implant at #18 with subgingival caries (Fig.1 *), because 4 of immediate implants have been and possibly need to be redone.  In contrast delayed implant at #19 fair the best with formation of the dense bone next to the coronal macrothreads (Fig.2 *) in spite of exposure of the microthreads (<).  When the tooth #18 is extracted, the socket opening appears to be narrow.  The lingual septum is narrow and short (possible C canal).  Fine particles of cortical allograft mixed with blood is placed in the mesial (M) and distal (D) sockets and covered by compressed collagen plug and Cytoplast.  PTFE suture is used.  The future implant should be small in diameter and placed subcrestal.  Acrylic dressing is in place and loose 2 weeks postop (Fig.3), presuming keeping Cytoplast in place.  The dressing is going to be removed 5 weeks postop.
