After adjustment of the trajectory of osteotomy (Fig.5), a 3x16(2) mm 1-piece implant is placed within normal limit (10-15 Ncm, Fig.6)).  In fact bone graft is placed before (Fig.6,7 arrowhead; to repair apical buccal plate perforation) and after (Fig.6 *) implantation.  选择一段式植体（3x16(2)毫米，图六），颊侧，舌侧骨板不容易穿孔，植体方向也容易控制（图七，在没有X光眼力下）。植骨分两部，植入植体前，放置少量骨粉修复根尖颊侧炎性穿孔（图六，七：箭头），第二次骨粉放置在植体植入之后，于植体周围（图六：*），主要颊侧间隙。
