Short and Soft Bone After Implant Removal   

A 48-year-old man returns for CT 4.5 months post infected implant removal with bone graft at #15.  The bone height reduces from 6-10 mm to ~ 4 mm (Fig.1).  To place a 5x7.3 mm implant (Fig.2), initial osteotomy for ~3 mm in depth and in diameter will be aided by guided surgery (Fig.3 (red line: sinus membrane)).  Internal sinus lift is going to be conducted with Magic Lifter free hand or in the middle of the tube (Fig.4 arrows).  The specially designed lifter is able to lift the sinus floor without tearing the membrane.  To prevent and repair membrane injury, one or 2 pieces of PRF membrane will be inserted underneath the lifted structures (Fig.5 yellow), followed by bone graft (green circles).  Then 3.5-4.5 mm taps will be used with guide to further expand and condense the osteotomy.  With more graft to be placed, the 5x7.3 mm implant will be placed (Fig.6 pink).
