After extraction of the tooth #2 (Fig.1), osteotomy is supposed to be initiated in the mesial slope (Fig.2 arrow). To prevent sliding, the osteotomy should be done by a drill first (lightly), followed by osteotomes to avoid sinus membrane perforation. Then the trajectory must change.
