Spacer for Socket Preservation (Case 2) M

A 55-year-old woman requests extraction of the tooth #2 with pain and mobility a few weeks post SRP (Fig.1) and finally agrees with socket preservation (Fig.2,3 *).  In fact the mesiobuccal root appears to have been fractured.  She is a heavy bruxer (nuts).  There is a large amount of granulation tissue in a large single socket.  After suture fixation over Osteogen plug and allograft (*), a prefabricated band (B) and loop (L) are cemented in place.  Periodontal dressing is applied mainly underneath the loop for wound protection and bone graft and membrane fixation.  The central portion of the periodontal dressing remains in place over the socket opening 1 week postop (Fig.4).  The removable distal portion of the loop dislodged 23 days postop.  The free end of the fixed mesial portion of the loop is irritating; the patient returns the following day.  After removal of the spacer, the socket appears to have healed (Fig.5).  No bone graft has been lost (Fig.6 *, as compared to immediately postop (Fig.2,3)).  Return to Upper Molar Immediate Implant, Trajectory Xin Wei, DDS, PhD, MS 1st edition 04/13/2019, last revision 01/06/2020
