The longitudinal fracture is not noted until postop (Fig.4 < (*: apical granulation tissue)).  Intraop finding is large amount of granulation tissue removed from the mesial apical region (Fig.5 *, hidden).  G: allograft for sinus lift.  4x10: IS dummy implant.
