Small Implant to Prevent Periimplantitis

A 60-year-old man develops periimplantitis at #3-5 (Fig.1).  After failure of two rounds of bone graft, the implant at #4 is removed with bone graft 3.5 months earlier.  The infection resolves.  Before the implant removal, there is no gingiva between #4 and 5.  It appears that these 3 implants are placed too close to each other and/or they are too large.  To prevent recurrent infection, a 2.5 or 3.0 mm implant will be placed.  Take preop PA.  Incision will be made.  If the bone is narrow buccopalatally, use 1 and 1.5 mm microosteotomes to initiate osteotomy.  Also prepare Tatum 1- and 2-piece implants (3.5 mm).
