A tissue-level tapered implant as wide and as long as possible, 4.5x20 mm, is placed (Fig.4 immediately postop).  The insertion torque is between 35 and 40 Ncm. 

To get better primary stability, a larger (5x20 mm) or longer (3.5x23 mm, the best available) should have been used so that the implant does not have to be placed so deep.  With the longest implant (3.5x23 mm), the sinus floor (cortical plate) is engaged and lifted.
