Offending Tooth

A 73-year-old woman has had discomfort with the upper left FPD for long time (Fig.1).  A large asymptomatic fistula is found recently above the pontic with a small superior pedicle (Fig.2,3 <).  CT shows radiopaque (Fig.1,4 *) and radiolucent (Fig.4 >) lesions are associated with the tooth #13, instead of the tooth #11.
