Immediate Provisional for Upper Canine
A 44-year-old man has a congenital missing canine. Maryland bridge fails. The mesiodistal space is 5 mm (Fig.1,2 *). A 3.0x17 mm one piece implant is placed (Fig.3 I: implant; A: abutment). When the implant is placed, the papillae appear to form instantly (Fig.4; A: prepared abutment). An immediate provisional is fabricated (Fig.5). There is no occlusal contact with the opposing in any excursion movement.
The provisional keeps the papillae in position and shape (Fig.6, one week postop).
The papillae remain healthy 5 months postop (Fig.7). The definitive restoration take over the task of holding the papillae in place (Fig.8,9, immediately post cementation).
There is no bone resorption 3 months postop (Fig.10), 9 and 14 month post cementation (Fig.11,12). The hard and soft tissues remain healthy 3 years 6 months post cementation (Fig.13,14). One thread appears to be exposed 5 years 4 months post cementation (Fig.15).
In fact, his son has the same condition: two missing canines.
