The osteotomy after using 3.8 mm bone expander is then intended to move mesially with Lindermann bur again and the depth is increased to 14 mm with sequential bone expansion with expanders until 3.8 mm.  Finally a 5.3x14 mm submerged implant is placed with insertion torque ~ 40 Ncm (Fig.3).  The tilting of the implant is due to the effort to move the implant mesially with Lindermann bur.  This time it does not work so efficiently.
