Dr. Wei, Regarding your case of
#30 reinfection ,
After I review your notes and a series of PA, I believe and
will do:
1) We do need to consider some issues of the perio furcation
radiolucency, tooth fracture but the progressively enlarged
radiolucency around mesial apexes with under shaping/cleanng
/ obturation strongly indicates RCT failure as a primary
cause of reinfection.
2) If I do this case, I will retreat #30 after careful IOE (intraoral
evaluation) and make sure that there is no poor perio
condition and fracture (tough and not easy) after removing
restoration.
3) If something happens during Tx, we need to inform patient
on consent beforehand "it may need extraction or root
amputation" . Both of alternative Tx have reported merit
results. I can share with you my similar cases I performed
in the past.
