Bone-level Implants

A 62-year-old man (CZ) has had chronic periodontitis.  A month ago, five immediate implants were placed in the lower anterior region.  Fig. 1 shows a PA taken 4 years ago: #4 residual root and #6 severe bone loss.  A 4.1x14 mm bone-level implant is planned immediately after extraction (Fig.2).  Clinically, the residual root is oval in shape (Fig.3 dashed line).  The implant is placed as palatal as possible after socket treatment with 2% Xylocaine/1:50,000 Epinephrine (Fig.4).  In addition, the osteotomy may have to be established in the distal wall, since the root tip tilts mesially (Fig.1).  Take the 1st intraop PA after 2 mm pilot drill.
