Do Not Perforate Sinus Floor Initially
A 64-year-old man will return for #14 extraction/implant due to palatal root fracture (Fig.1,2). He has a history of cheek bite when the teeth #1-3 are replaced by implant crowns. His models for these crowns have been found. Alginate impression has been taken for the arches so that the new crown will be fabricated as close as possible to his original one. Take preop photos to show palatal root fracture and overjet buccally. Initiate osteotomy in the septum slightly buccally due to the palatal bone defect; the depth will be 8.5 mm (Fig.1,2). Use DIO Bone Expanders or an implant to break the sinus floor (Fig.3). The implant will be placed slightly apical to the crest.
