The gingiva-level implant is slightly subgingival except buccal (Fig.5 mirror image). To facilitate buccal gingival re-growth to cover the exposed implant surface, bone graft is placed subgingival (^). P: palatal gingival margin (direct view).
The implant turns out to be large and placed deep to achieve primary stability. The drawback is difficult to fix the buccal gingival recession. The initial osteotomy should have been placed more palatal.
When the lesion (diseased tooth) is removed, the gingival margin will most likely return to the normal level.
