Palatal Root Fracture Buccal Placement

A 53-year-old woman is probably a bruxer.  The tooth #15 has been symptomatic in the last few years.  The palatal root appears to have vertical fracture.  The palatal fragment seems to migrate palatally.  She is reluctant to receive treatment.  Recently an abscess develops mesiopalatally (radiolucency around the palatal root: Fig.2 as compared Fig.1 (taken 5 years 9 months earlier)).  After extraction without local application of Antibiotic, osteotomy will be initiated as buccal as possibe and as long as possible (penetrating the sinus floor, Fig.3).  If primary stability is questionable, fabricate no or short immediate provisional with ample clearance.
