Expanders for Upper 2nd Molar Implant
A 49-year-old lady has history of periodontal disease (Fig.1). The teeth #2 and 19 have been extracted and replaced by immediate implants. When she returns, crown is going to be cemented at #19 and delayed implant will be placed at #15. The tooth was extracted 4.5 years ago. No PA has been taken since extraction. Fig.2,3 show design for immediate implant if the tooth were not extracted (*: mesial crest; O: bone graft). Note the orientation of the implant and the space from the neighboring tooth.
#15 blade will be used for access and periosteal elevator for opening osteotomy site. Set implant spacer at 4 mm. Since the maxillary posterior region usually has poor bone density, bone expander kit will be most likely used until 2-3 sizes smaller than the expected implant. The latter will be decided by preop and intraop X-ray.
An abutment will be placed if indicated. There may be a vertical issue considering extraction long time ago. The opposing tooth may be adjusted prior to an immediate provisional.
