Supraeruption and Intrusion
A 56-year-old man has long-termed partial edentulism (#2,5,14,15 (not shown),29, Fig.1). There is supraeruption of the opposing teeth (Fig.1 arrows). If an immediate provisional is provided, it should have plenty of clearance to avoid micromovement of the implant. Once the implants at #2,5, and 29 osteointegrate, they will be used as anchorage for intrusion (Fig.2,3 arrows, segmental orthodontics).
The first implant will be placed at #29 (Fig.4,5). The pointed ridge will be flattened to start osteotomy (Fig.6,7 (surgical handpiece)). The buccolingual dimension of the flattened ridge should be ~ 6 mm. The initial depth will be 12 mm. Take the 1st PA. A restorative high speed handpiece may be needed to remove the enamel of the mesial surface of #30 and the distal surface of #28 to increase space for implant placement.
