The fact of the narrow ridge (Fig.6) is also ignored intraop.  Mainly there is no idea to how to handle the narrow ridge.  When the implant are placed, the buccal surface of the implants is exposed.  What is done is to decorticate the buccal plate (which is apparently related to postop infection and bone graft loss) and to placed autogenous bone graft over the exposed implant surface.  *: osteotomy site at #19.  A parallel pin is placed at #18.
