Lower 1st Molar with Three Roots
A 68-year-old lady (WQ) had CT taken 3 years ago (Fig.1-3), which show 3 roots (M: mesial; DB: distobuccal; DL: distolingual) of the lower right 1st molar and large caries (C). Recently the tooth fractures after root canal therapy (Fig.4-6).
Having 3 roots suggests a tripod septum. Surgical handpiece and elevators may be required, in addition to proximators. Cowhorn forceps should be helpful. The socket will be treated with Clindamycin, selected by time sequence.
An implant placed the tripod septum should be stable. The length of the initial osteotomy is 12 mm using 2 mm pilot drill. Take intraop PA as early as possible for trajectory and depth determination.
