From Large Radiolucency to Crown Fracture

A 55-year-old man must be a dental phobic.  With a fistula (Fig.1 <) and large radiolucency associated with the lower left 2nd molar, he postponed root canal therapy for 2 years and 8 months (Fig.2).  Later he refused to have crown for the tooth.  Three years and 5 months later, the radiolucency seems to have been resolved (Fig.3), but the tooth has fractured with buccal infection.  The patient is unwilling to accept treatment of extraction (no antibiotic socket treatment) and immediate implant (Fig.4).
