Fig.3: Implant design: 7x14 mm tapered.  The largest mesiodistal width of the root is 9 mm.  A vertical root fracture is noted in the mesial root (Fig.4: lingual view), consistent with the gutta percha pointing to the mesial root (Fig.3).  The post in the mesial root is the culprit.
