After ridge split with wheels and #15 blade as deep as possible, bone expansion is carried out using 1 and 1.5 mm micro-osteotomes for 11 mm (Fig.4 pink line) and 2 mm RT for 8 mm.  A 1-piece implant is placed (3x14 mm or narrower,  Fig.5 green).  The buccal plate may be displaced.  Raise the buccal and palatal flaps to expose the narrow portion of the ridge.  Do not over expose the bone in case of microfracture.

Keep the 1st or 2nd micro-osteotome in place (separate from the handle, Fig.4) for PA.
