A 3 mm drill is left in place with 18 mm subcrestal lingually (Fig.5 L).  While CT is being taken, there is hemorrhage, probably from the Incisive Canal (I).  Osteotomy has been created in the native bone for 4.8 mm.

When the 3 mm drill is withdrawn, a 3.8x16 mm implant is placed ~ 2 mm subcrestal lingually to reduce buccal thread exposure.
