Mesiolinguoapical Placement
A 53-year-old woman has buccal gingival erythema and edema with deep pocket and purulent discharge. PA and CBCT show that the implant was placed distal and not deep enough (Fig.1,2) as well as buccal (Fig.3). When the failed implant is removed, try to place an implant mesiolinguoapically. Make purchase point/trough mesiolingual to the failed implant. Take PA with pilot drill for trajectory confirmation (Fig.1 red line). The implant will be 4 or 4.5 mm in diameter and 4 mm in the native apical bone. Use 1.6 and 3.8x11 mm Magic Drill until the lingual crest. Place PRF over the bone graft, Cyotplast buccally and provisional.
