Since the preop ridge looks narrow, incision is made, which makes it easy for observation of osteotomy. First, a 10.5 mm bone trimmer makes a deep dimple with apparently moderate ridge for a 4 mm implant. The patient has hypertension and cannot lie down for long. The bone density feels not high; a 4x11 mm (instead of 9 mm) implant is placed for 10 mm osteotomy (Fig.1 (~ 35 Ncm)). After use of 6 mm bone profile drill, there is bone buccal (less) and lingual to the implant. A 5.5x4 mm healing abutment is inserted, barely supragingival.
