10.5 mm Bone Trimmer

Since the preop ridge looks narrow at #30, incision is made, which makes it easy for observation of osteotomy.  First, a 10.5 mm bone trimmer makes a deep dimple with apparently moderate ridge wide enough for a 4 mm implant.  The patient has hypertension and cannot lie down for long.  The bone density feels not high; a 4x11 mm (instead of 9 mm) implant is placed for 10 mm osteotomy (Fig.1 (~ 35 Ncm)).  After use of 6 mm bone profile drill, there is bone buccal (less) and lingual to the implant.  A 5.5x4 mm healing abutment is inserted, barely supragingival.  As the implant is 1 mm longer than the design, regional panoramic X-ray is taken to determine clearance from the Inferior Alveolar Canal (Fig.2 red dashed line (the upper border).  There is crestal bone loss 4 months postop (Fig.3,4).  It appears that the implant was placed excessively deep.  There is no implant thread exposure 9 months post cementation (Fig.5,6).
