Free Hand Placement in Narrow Ridge
A 60-year-old woman returns for CT 4.5 months post implant removal at #6 (Fig.1). The ridge remains narrow and short because of the impacted tooth. The latter appears necessary to be extracted for a longer implant (12 or 14 mm vs. 10 mm). To accommodate a larger implant (2.5 mm vs. 2.0 mm), prepare DIO bone expander kit. But first, use an initial drill from IS Mini implant kit, followed by a parallel pin for trajectory and occlusal check. Take intraop CBCT if needed. Osteotomy will be initiated palatal (Fig.3 *, as compared to the previous osteotomy (Fig.2 <)). To reduce palatal implant thread exposure, use an implant with 4 mm cuff and place bone graft/collagen membrane. At first take impression with Alginate for wax up. In fact the stent is available. Instead PVS impression is taken for guide. The latter will be used to start initial osteotomy (2.2 mm drill for a few mm (4)), followed by 1.2 mm drill for depth.
