Short Implant Placed Deep
The clearance between the residual roots of the tooth #3 and the abutment at #30 seems to be somewhat limited (Fig.1). The initial osteotomy depth is 10 mm (Fig.2). After moving the osteotomy mesially, the depth increases to 12 mm; a parallel pin is inserted for 16 mm (Fig.3 with sinus membrane perforation). The depth of the rest of the osteotomy is 10 mm with placement of a 5x10 mm implant with >50 Ncm (Fig.4); with insertion of a 6.8x4(3) mm abutment and Vera Graft (*), an immediate provisional is fabricated. Collagen plug is used to seal the gap between the provisional and the gingiva. The abutment dislodges nearly 1 month postop; the wound has healed (Fig.5). A 8.2x5(4) mm healing abutment is placed to keep the soft tissue configurement.
