Short, Fat Implant for Crack Tooth
A 58-year-old woman cannot masticate on the left. The tooth #15 has a mesiodistal crack line (now completely split; take preop photo) with mild percussion and mobility I. Pulpal test shows necrosis. Although the roots seem fused and short, they are curved, suggesting bruxism (Fig.1). Use 3 mm Magic Expander to initiate osteotomy (Fig.2 green line) in the mesial slope of the socket (red dashed line). If the bone is dense, switch to Pointed/Lindamann bur. Sinus lift appears to be imminent. Draw blood before the doctor shows up. The implant will be most likely short and fat (5x8.5 mm).
Since IS implants are more or less cylindrical and if a 5 mm one does not achieve primary stability, a larger Tatum tapered tap should be tried immediately without using IS cortical taps.
