Occlusal Trauma

A 74-year-old woman requests treatment for a loose, tender tooth in the upper right quadrant (Fig.1).  It may be due to occlusal trauma from the opposing implant tooth (Fig.2 arrow) and endo failure (Fig.2 coronal section).  Because of  history of low bone density and buccal placement at #7 with low primary stability, a small diameter implant will be placed palatally at #5.  Either a 4x13 mm bone-level (Fig.3) or 4x17 mm tissue-level implant will be placed.
