Sinus Lift with Dummy Implants
After flattening the root stump with surgical burs and bone trimmer at #4 (Fig.1), it is difficult to use point drill and 2.2x7.3 mm drill because of incomplete engagement of drill cylinder with guide sleeve (Fig.2). The initial osteotomy seems to be buccal. With removal of the remaining root, it is much easier to finish osteotomy (better engagement of drill cylinder with the sleeve) and sinus lift with dummy implants (Fig.3,4 (*: bone graft)). Finally a 4.5x10 mm implant is placed (basically in the middle of the socket with even buccal and palatal gaps) with primary stability and a cemented abutment is hand tightened (after 5.5 mm profile drill) for an immediate provisional (Fig.5). The gingiva is healthy around the provisional with almost perfect seal 2 weeks postop (Fig.6 P). There is no obvious bone loss 7 months postop (Fig.7). There is apparent mature bone in the sinus 9 months postop (Fig.8 *).
