After pushing the mesial mucoperiosteal flap (F in Fig.15) distally,  an osteotome (O in Fig.16) is placed between #9 and the implant to make the first vertical bone cut.  Initially a thin and narrow osteotome is used.  Gradually thicker and wider osteotomes are used.
